Summary:Prostatic stromal sarcoma (PSS) is an exceptionally rare mesenchymal tumor of the prostate. We present a case of high-grade recurrent PSS in a 49-year-old man, managed with complete surgical excision and successful organ preservation. This report is accompanied by a literature review highlighting diagnostic challenges, histopathological features, and evolving surgical approaches, including robotic-assisted techniques. By comparing our case with previously reported cases, we emphasize the importance of individualized treatment strategies and long-term follow-up in managing this aggressive malignancy.
