Pochodzą od króla Wysp (''Lord of the Isles'') Somerleda, który w połowie XIII wieku władał Hebrydami. Przodkiem Macdonaldów jest Ranald, młodszy syn Somerleda, od którego bierze nazwę gałąź Macdonaldów of Clanranald. Nazwisko wywodzi się od syna Ranalda, imieniem Donald. Od starszego syna Somerleda, Dougalla pochodzi inny klan klan MacDougall.
Do połowy XV wieku panowali prawie niezależnie od królów Szkocji, używając tytułu ''Lord of the Isles'' – Panów Wysp. Po masakrze w Glencoe w 1692 roku, w której zginęło 70 członków gałęzi klanu MacIain, Macdonaldowie poparli stronę jakobitów w powstaniu w 1715 roku, ale w czasie powstania Bonnie Prince Charliego w 1745 roku klan był podzielony.
